Grade I ankle sprains need to be protected
without any restriction in activity for prolonged periods. Often,
we will place the ankle in a clamshell brace and allow normal walking.
Sports activity is protected for two to three weeks with any side
to side motion and physical therapy is done if pain is not much
              better two weeks after injury.

Grade II ankle sprains need added protection. Often we will place
the foot in a protective boot with a soft wrapping to allow the
ligaments to heal. Range of motion is started at one week after
injury with physical therapy exercises. No side to side motion is
done for two weeks. A clamshell brace is used at two weeks and increase
in therapy is allowed. Full return to activity is at 4 to 6 weeks
depending on the level of injury and time for recovery.Grade III
ankle sprains require extensive therapy and protection. The ankle
is wrapped in a soft cast and a protective boot for 2 weeks.

Weightbearing is begun when the patient is
comfortable. Full weight can be placed on the ankle with comfort.
At two weeks, physical therapy exercises are done to increase
range of motion, decrease pain and inflammation and increase stability.
No side to side motion is permitted for one month and average
return to activity is at 6 to 8 weeks. An ankle brace is worn
for 3 months following injury during sports.
